Emily Amerman has many years of leadership in the field of aging. She has been involved with the evolution of community-based long term care models over the last three decades. She is currently Executive Director of LIFE St. Mary (Living Independently For Elders), a Program of All-inclusive Care for the Elderly (PACE). LIFE is sponsored by St. Mary Medical Center. Formerly Ms Amerman was the founding Executive Director of St. Agnes LIFE (now known as Mercy LIFE), sponsored by St. Agnes Continuing Care Center in Philadelphia, PA. Ms. Amerman has held positions with the Philadelphia Geriatric Center, the Philadelphia Corporation for Aging (PCA), and Genesis HealthCare. At PCA, Ms. Amerman implemented the PDA Waiver, the In-Home Support Program, the Family Caregiver Support Program and the start-up of Older Adult Protective Services. Ms. Amerman has a Master’s degree in Social Work (MSW) from the University of Pennsylvania and a Bachelor’s degree from New York University. She has been involved with PCMI since its inception, and she also serves as a director of the Center for Advocacy of the Rights and Interests of the Elderly (CARIE).
Linda Forest completed her undergraduate education at The Pennsylvania State University and received an M.S. from Villanova University. She has been employed by Montgomery County Aging and Adult Services for 32 years. Throughout her career, Linda has had the unique experience of witnessing the dramatic expansion of the Area Agency on Aging. When she was hired by Aging and Adult Services the agency had been in existence for only 3 years and there were 12 full-time staff. The agency now has four regional offices and over 100 employees. Linda began her career as a care manager and, over the years, has held various supervisory roles. She has been the Clinical Deputy Director since 2001.
Pearl Graub, a leader in the field of gerontological social work, has over 25 years of clinical and administrative experience in Community Based Long Term Care, hospital social work, care management and aging. She currently is Assistant Director of Long Term Care Options for the Philadelphia Corporation for Aging, the Area Agency on Aging for Philadelphia County. Prior to this role, she was the Director of Professional Services and Director of the Pennsylvania Department of Aging Waiver Program, the Options Program and the Philadelphia Channeling Project, part of the National Long Term Care Channeling Demonstration. She has been involved in several joint initiatives between hospital-based systems, managed care systems, behavioral health services and the Philadelphia Corporation for Aging. Ms. Graub was a founder of the first ethics committee at an AAA in a Community Based Long Term Care setting at the Philadelphia Corporation for Aging. She was the Project Director for the development of six Care Management Quality Assurance Clinical Protocols, which later were expanded to 13 with training videos. Ms. Graub co-authored a chapter on Community Based Long Term Care: A Collaborative Approach in the book Handbook of Home Health Care Administration by Marilyn D. Harris. Ms. Graub holds a Bachelor of Social work from Syracuse University and a Master of Science in Social Work and Gerontology from Columbia University in New York.
Denise V. Stewart, is the Deputy Director of Long Term Care at the Delaware County Office of Services for the Aging (COSA). She is a graduate of West Chester University and holds a Masters Degree of Social Work Administration from Temple University. She has been active in the field of aging for over 30 years. Her work experience over the past 30 years her includes working as a Care Manager for COSA, at an Home Care agency as a Home Care Department Manager, Admissions Director and Social Work Director at an area nursing home, Social Worker at an area hospital, Supervisor and Home and Community Based Services Program Director at COSA prior to her becoming the Deputy Director in 2001. She serves on numerous Advisory Boards and Board of Directors. Her Board of Directors participation includes being the President of the Professional Care Management Institute (PCMI), Vice Chair of the Delaware County Women’s Commission and a member of the Mental Health and Aging Network (MHAN) Leadership Council (ASA) and the Network of Multicultural Aging (NOMA) Leadership Council (ASA). Currently, she serves on the American Society on Aging (ASA) East Coast Conference Planning Committee and the PA Office of Mental Health & Substance Abuse Services Older Adult Committee (OMHSAS), where she co-chaired for 8 years.
Ms. Tobin has been a social worker in the field of aging for 30 years. She is committed to the welfare of homebound older adults and to the advancement of the professional skills of those working with them. Having served as a Caseworker and a Supervisor at the Bucks County Area Agency on Aging (AAA), Ms Tobin is now a Deputy Director there, responsible for the Long Term Care Unit which provides care management and services to homebound older adults in Bucks County, and for the services provided to AAA clients by its contracted service providers. She also worked as an independent consultant providing training to social workers in aging for 10 years. Ms Tobin is Secretary of the Board of Directors of the Professional Care Management Institute (PCMI), member of the PA Long Term Living Training Institute Advisory Committee, and the National Association of Social Workers.
